The school survey opens on February 9, 2026 and will close on April 1, 2026.
- It is important that you take the time and fill out this survey as your voice is integral in our schools and your input is valued.
- If you have more than one child attending P.S. 33, you will need to fill out a survey for each child.
- There will be two ways you can fill out this survey, online or a hardcopy that will be sent home with your child.
- For the online portion, you are not receiving survey access codes this year as you will use your child's OSIS number to access the survey. Please enter a lower case "f" followed by your child's nine-digit Student Identification (OSIS) Number to access the survey (e.g. f123456789).
- Student OSIS numbers can be found on student report cards or in the family's NYC Schools Account. To access your families NYC Schools Account please visit https://www.schoolsaccount.nyc/
- To fill out the survey online please click the Take the 2026 NYC School Survey button above.
