NYC Department of Education Health Screening
Since the NYC DOE Health Screening is updated from time to time, we are strongly encouraging you to use the online Health Screening on your phones or tablets rather than the paper form.  The online Health Screening is the most up to date screening to enter our school building.  Using the online form will also ensure for a quicker arrival during these colder months and inclement weather.  Remember you will be required to show proof your child has passed the health screening every day upon arrival.
