Join us at the Office of Family and Community Empowerment (FACE) for Supporting Early Math at Home, a workshop series designed for parents and caregivers! Discover everyday strategies to make learning early math fun and engaging!
This series will provide you with hands-on activities, expert insight, and resources to help build your child’s confidence, skills, and love for math at home!

Please complete a 2024-2025 Family Income Inquiry Form (formerly known as School Meals Form) by December 6, 2024.
This form must be completed every year.

Submit only one form for all children in the household attending school in grades PreK through High School.
You can complete the form from any computer, tablet or smartphone with an internet connection.
Completing your form online is quick and easy.
Families, regardless of income, are asked to complete a 2024-2025 Family Income Inquiry Form (formerly known as School Meals Form) so that our school can receive additional funding.
You do not have to give any income information if you are certain that you don't qualify, however the NYC DOE asks that families submit the form. If you do qualify, you only need to provide one income, preferably the lowest income of the household.

Pandemic Electronic Benefit Transfer Program (P-EBT) Food Benefits
The Pandemic Electronic Benefit Transfer Program (P-EBT) is a Federal program to provide food benefits to children who miss the opportunity to receive school meals due to pandemic-related school closures. These benefits will take the form of funds on electronic benefit transfer (EBT cards) which can be used in the same manner as SNAP (food stamp) cards.
All children at DOE schools are eligible.

- Benefits will automatically be issued to all eligible students in April 2023 and May 2023, program enrollment is not required.
- For students whose households receive SNAP, Temporary Assistance, or Medicaid benefits, P-EBT benefits will be loaded to those household accounts.
- For all other students, new cards will be issued to each student specifically for P-EBT benefits and mailed directly to students’ home addresses.
