NYC DOE Consent For Covid-19 Testing
When we return to in-person learning on December 7, 2020, staff and students from grades 1-12 will be participating in weekly random testing for COVID-19 through the rest of the school year. Regular testing of staff and students in schools allows us to keep our schools and broader communities healthy and safe.

In order for your child to return to in-person learning, you must submit the consent form for in-school COVID-19 testing by the first day your child returns to their school building.

Visit the NYC Department of Education Covid-19 Testing for Students and Staff page for more information on how to submit a consent form online or to print copies of the form to give to your child's teacher.
Please complete your Daily Health Screening Prior to coming to school daily. You may access it at NYC DOE Health Screening.

You will be required to show proof your child has passed the health screening every day upon entry.
